CNA Coordinator - Hospice

Responsibilities

• Allocate resources to increase staff to patient ratios.
• Increase clinical visits while lowering clinician caseloads.
• Assist with staffing assignments for new admissions and scheduling.
• Respond to phone calls from all disciplines, family members and referral sources.
• Follow up with staff to provide support and education as needed.
• Avoid missing patient visits and ensure the daily schedule of patients is complete and updated in the database.

• Complete weekly reports which will be addressed in a weekly meeting with direct supervisor.

Requirements

• Previous CNA leadership and/or hospice experience.
• Ability to work well within a team environment but also able to work independently.
• Excellent communication skills.

Qualifications

• Strong familiarity with computer applications and spreadsheets.
