Facility: Good Samaritan – Woodland, a 42-bed long-term skilled care and transitional care facility in Brainerd, Minnesota (“The Heart of the Brainerd Lakes Area”).

About the role

The Nursing Assistant (NA) serves as caregiver to residents during the scheduled work period in long-term care. Provides resident-centered nursing care and daily living assistance under the supervision of a registered nurse (RN). Delivers age-appropriate care related to the physical and psychological needs of residents as per care plan. A member of the nursing team, accountable for following infection prevention and control policies and Personal Protective Equipment use.

Responsibilities

  • Assist with basic health care needs and daily living activities: bathing, toileting, grooming, dressing/undressing, obtaining and recording vital signs, and providing psychosocial support.
  • Help residents with transferring, repositioning, and walking using correct techniques and equipment; provide range of motion and passive exercises.
  • Complete tasks related to food preparation and dining, creating a positive dining experience; assist with meals, snacks, water, and supplements.
  • Aid providers and nursing staff with procedures as needed.
  • Document resident interactions, care outcomes, behavior, activity, and dietary information.
  • Communicate residents’ changing conditions and care concerns to the charge nurse.
  • Collect non-invasive body fluid specimens and gather vital signs.
  • Provide housekeeping and laundry services to keep residents’ living areas clean and operational.
  • Assist with and document wellness and recreational programs; encourage resident participation.
  • May participate in departmental or organizational committees and task forces.

Requirements

  • High school graduate or GED preferred, but not required.
  • Minimum age of 16.
  • Prior clinical or nursing assistant experience in long-term care preferred.
  • Certified Nursing Assistant (CNA) certification with the State Board of Nursing, or ability to become certified within four months of hire by enrolling in a training program or obtaining certification through endorsement (training and certification provided by facility).
  • Satisfactorily complete yearly department-specific competency testing and maintain continuing education credits (minimum 12 hours annually).
  • Maintain all required department-specific certifications.
  • When applicable, certified and registered with the Unlicensed Assistive Person (UAP) Registry.
  • For roles requiring driving, must meet Sanford Category II Driver requirements: valid driver’s license, motor vehicle report, and medical qualification per Sanford Driving Policy.

Schedule

  • Part-time: 32 hours per bi-weekly pay period (16 hours weekly).
  • Night shift: 10:00 PM to 6:30 AM.
  • Every other weekend and rotating holidays.
  • Additional incentives: $1.25/hour for weekend shifts and $1.25/hour for hours worked between 6:00 PM and 6:00 AM.
  • Sign-on bonus available if eligibility requirements are met.

Pay

Salary range: $20.00 – $25.50 per hour.
