Facility: Good Samaritan – Woodland, a 42-bed long-term skilled care and transitional care facility in Brainerd, Minnesota (“The Heart of the Brainerd Lakes Area”).
About the role
The Nursing Assistant (NA) serves as caregiver to residents during the scheduled work period in long-term care. Provides resident-centered nursing care and daily living assistance under the supervision of a registered nurse (RN). Delivers age-appropriate care related to physical and psychological needs as per the care plan. A member of the nursing team, accountable for following infection prevention and control policies and Personal Protective Equipment use.
Responsibilities
- Assist with basic health care needs and daily living activities: bathing, toileting, grooming, dressing/undressing, obtaining and recording vital signs, and providing psychosocial support.
- Help residents with transferring, repositioning, and walking using correct techniques and equipment; provide range of motion and passive exercises.
- Prepare and serve meals and snacks, provide water and supplements, and create a positive dining experience.
- Assist providers and nursing staff with procedures as needed.
- Document resident interactions, care outcomes, behavior, activity, and dietary information.
- Communicate resident’s changing condition and care concerns to the charge nurse.
- Collect non-invasive body fluid specimens and gather vital signs.
- Provide housekeeping and laundry services to keep residents’ living areas clean.
- Assist with and document wellness and recreational programs; encourage resident participation.
- May participate in departmental or organizational committees and task forces.
Requirements
- High school graduate or GED preferred, but not required.
- Minimum age of 16.
- Prior clinical or nursing assistant experience in long-term care preferred.
- Certified Nursing Assistant (CNA) certification with the State Board of Nursing, or ability to become certified within four months of hire (facility provides training and certification).
- Satisfactorily complete yearly competency testing and maintain continuing education credits (minimum 12 hours annually).
- Maintain all required certifications, including UAP Registry where applicable.
- For applicable roles, must qualify as a Sanford Category II Driver: valid driver’s license, motor vehicle report, and medical qualification per Sanford Driving Policy.
Schedule
- Full-time, 69.75 hours per bi-weekly pay period (34.88 hours weekly).
- Day shift: 6:00 AM to 2:15 PM.
- Every other weekend and rotating holidays.
- Additional weekend incentive: $1.25 per hour.
- Additional shift incentive: $1.25 per hour for any hours worked between 6:00 PM and 6:00 AM.
Pay
- Salary range: $20.00 – $25.50 per hour.
- $5,000 sign-on bonus (eligibility requirements apply).
