Essential Duties And Responsibilities
- Performs basic nursing care procedures under the direction and supervision of a licensed nurse.
- Follows treatment plan that meets the appropriate age, needs and abilities of each resident/patient, including assisting with meals and nourishment, transferring using proper body mechanics and appropriate assistive devices, ambulation, bathing, toileting and dressing and grooming.
- Observes and communicates pertinent information about the adult/older adult resident/patient’s condition to the appropriate facility staff.
- Takes and records pulse, respirations, blood pressures and weight as needed.
- Documents care and observations on work sheets and EMR accurately and in a timely manner.
- Maintains cooperative relationships to enhance the efficient operation of the care team by providing support and assistance to co-workers.
- Adheres to infection control practices.
- Supports the psychosocial needs of the residents as per the plan of care and profile.
- Participates in educational and inservice meetings to update knowledge of techniques, guidelines and regulations.
- Must attend all mandatory programs.
- Mets basic requirements for 12 hours of continuing education per year.
- Uses equipment in an appropriate and safe manner.
- Adheres to standards of the Diligent Minimal Lift Program.
- Follows facility policies and procedures.
- Performs other duties as required.
Minimum Qualifications
- High School diploma or GED preferred
- 6 months – 1 year experience as a CNA preferred
- State of Connecticut CNA Certification; or within 120 days of CNA Certification course completion
