About the role

Good Samaritan – Bethany is a 79-bed long-term skilled care and transitional care facility located in Brainerd, Minnesota, in an area known as “The Heart of the Brainerd Lakes Area.”

• Evening shift: 2:00 pm to 10:30 pm
• Rotating weekends and holidays
• Additional weekend incentive of $1.25 per hour
• Additional shift incentive of $1.25 for any hours worked from 6:00 pm to 6:00 am
• $5,000 sign-on bonus (if eligibility requirements are met)

Responsibilities

• Provide resident-centered nursing care and daily living assistance under the supervision of a registered nurse (RN).
• Deliver age-appropriate care related to the physical and psychological needs of the resident as per care plan.
• Assist with basic health care needs, including bathing, toileting, grooming, dressing/undressing, and obtaining and recording vital signs.
• Provide psychosocial support and personal care to assigned residents.
• Assist residents in transferring, repositioning, and walking using correct and appropriate transfer techniques and equipment.
• Provide range of motion and passive exercises.
• Prepare food and assist with dining, creating a positive experience for residents.
• Assist residents with meals and snacks, provide water and supplements as appropriate.
• Aid providers and nursing staff with procedures as needed.
• Document resident interactions, outcomes related to care, behavior, activity, and dietary.
• Communicate resident’s changing condition and care-related concerns/responses to the charge nurse.
• Collect non-invasive body fluid specimens and gather vital signs.
• Provide housekeeping and laundry services to assist residents with clean living areas.
• Assist with and document wellness and recreational programs, encouraging resident participation.
• May participate in departmental/organization committees and task forces.

Requirements

• High school graduate or GED preferred, but not required.
• Minimum age of 16.
• Prior clinical or nursing assistant experience in long-term care preferred.
• Certified Nursing Assistant (CNA) certification with the State Board of Nursing, or ability to become certified within four months of hire by enrolling in a training program or obtaining certification through endorsement.
• CNA training and certification provided by the facility.
• Satisfactorily complete yearly department-specific competency testing and maintain continuing education credits (minimum 12 hours annually).
• Maintain all department-specific required certifications.
• When applicable, certified and registered with the Unlicensed Assistive Person (UAP) Registry.
• For applicable states, may require a motor vehicle report and proof of valid driver’s license if role classifies as a Sanford Category II Driver.
• Must be medically qualified to perform essential driving functions per Sanford Driving Policy if required.

Pay

• Salary range: $20.00 - $25.50 per hour
• $5,000 sign-on bonus (if eligibility requirements are met)

Schedule

• Full-time, 32 hours per week (64 hours per bi-weekly pay period)
• Evening shift: 2:00 pm to 10:30 pm
• Rotating weekends and holidays
