Job Summary
The CNA provides direct care to residents while under the supervision of a licensed nurse, RN or LPN. The Care Center CNA provides care for adult and geriatric patients in the Madison Healthcare Services’ skilled nursing facility.
Duties/Responsibilities
- Provides direct care services for residents at the Care Center, including meeting residents’ needs in the following areas:
- Dressing, bathing and grooming
- Food and fluids
- Exercise and walking
- Elimination
- Mental stimulation
- Vital signs and weight
- Filling supplies in residents’ personal care basins in their rooms.
- Aids residents to attain highest degree of functional independence.
- Communicates with residents in a kind and caring manner; listens to their concerns and gathers necessary information.
- Takes immediate action to meet residents’ requests or needs.
- Assists residents to take tub baths and nail care.
- Auxiliary support to the Therapy Department as time permits.
- Assists with helping residents eat at mealtimes.
- Listens to reports at the beginning of each shift, clarifies assignment, reviews care plan/Kardex for assigned residents.
- Suggests change in plan as needed.
- Aware of appropriate outlined care plan and incorporates them into routine for the shift.
- Observes and reports resident changes to the charge nurse.
- Completes residents ADLs in Point Click Care including appetite, target behaviors, bowel movements, etc.
- Participates in performance improvement teams as requested; suggests recommendation to improve quality of care.
- Attends and actively participates in department meetings.
- If unable to attend, obtains information from review of meeting minutes.
- Alters practice when need to do so indicated by results of PI/QA monitoring.
- Maintains continuing education requirement for registry.
- Attends mandatory in-services.
- Performs work safely, without causing harm or risk to self, others or property.
Qualifications
- Current registered or able to register as a Nursing Assistant in the State of MN

Pay
$20-$27
Schedule
An employee who is not regularly scheduled. PRN/Casual employees in the nursing department (CNA/NAR, TMA, LPN, RN) will pick up no less than 24 hours every month with 8 of those hours being on the weekend and 16 of those being on a weekday. PRN/Casual employees will not be entitled to the incentive pay for the weekday hours and will work at their regular rate of pay with shift differential. Weekend Incentive Pay- Friday 2:00 PM to Sunday 10:30 PM. Shift Differentials: Evening: $1.50 Night $2.00
