Position Summary

About the Role

The Clinician Support Specialist enhances the performance of the clinical care team by supporting and facilitating clinical services with their assigned providers. They work in the field with the assigned provider as directed by Lead Clinician Support Specialists or Managers/Supervisors. Travel to facilities up to 60 miles from home is possible.

Responsibilities

  • Ensure the clinical provider in the assigned building can focus on patient care.
  • Facilitate proper patient management and document encounters for CCM and BHI services.
  • Schedule rosters and update them daily to ensure accuracy and timely delivery.
  • Avoid last-minute changes to scheduled visits and assist with day-of changes due to acute needs, hospitalizations, admissions, or discharges.
  • Complete opening encounter notes, medication reconciliations, and enter facility chart data into patient encounter notes.
  • Assist with collecting and sorting clinical paperwork and ensuring its completion each day.
  • Host telehealth sessions as assigned by manager/supervisor.
  • Share patient schedules with key facility staff as needed.
  • Aid in triage needs and ensure all needed paperwork is routed for the facility to Eventus providers, office, or families.
  • Assist with discharge planning and ensure check-ins/out with facility staff.
  • Maintain patient confidentiality and communicate transitions of care to CCM team.

Qualifications

  • High School Diploma or equivalency.
  • Valid driver's license and reliable transportation.
  • Experience in medical scribing preferred.
  • Typing speed of 45 wpm or greater.
  • Clinician Support Specialist certification a plus.
