Clinician Support Specialist
Position Summary

About the Role
The Clinician Support Specialist enhances the performance of the clinical care team by supporting and facilitating clinical services with assigned providers. They work in the field with the assigned provider as directed by Lead Clinician Support Specialists or Managers/Supervisors. Travel to facilities up to 60 miles from home address may be required.
Responsibilities
- Ensure the clinical provider is focused on patient care.
- Serve as a buffer between the provider and families/staff to manage patient care effectively.
- Document appropriate encounters and time for CCM and BHI services.
- Schedule rosters and update them daily to ensure accuracy and timely delivery.
- Avoid last-minute changes to scheduled visits due to acute needs, hospitalizations, admissions, or discharges.
- Complete basic opening of encounter notes according to discipline protocols.
- Conduct medication reconciliations on all scheduled patients during each encounter.
- Enter facility chart data and assign to providers as per guidelines.
- Aid in the collection and sorting of clinical paperwork for provider review.
- Host telehealth sessions as assigned by manager/supervisor, both routine and acute visits.
- Share patient schedules with key facility staff as needed.
- Triage any urgent needs as directed by assigned provider.
- Aid in the discharge planning of clinical services as needed.
- Report daily patient status to facility staff.
- Maintain patient confidentiality at all times.
- Communicate transitions of care to the CCM team to enhance patient care and education.
Qualifications
- High School Diploma or equivalency.
- Valid driver's license and reliable transportation.
- Experience in medical scribing preferred.
- Typing speed of 45 wpm or greater.
- Clinician Support Specialist certification a plus.
