Position Summary

Eventus WholeHealth delivers an integrated model providing holistic care for medically vulnerable adults, achieving extraordinary outcomes. Services include medical care in skilled nursing, assisted living facilities, and private residences through a network of healthcare providers. The Clinician Support Specialist enhances clinical performance by supporting and facilitating services with assigned providers.

About the Role

You may be asked to travel up to 60 miles from your home address. Hours: Monday to Friday - 8-hour day shift. Fuel card provided to assist with commuting.

Responsibilities

  • Ensure the clinical provider can focus on patient care.
  • Serve as a buffer between the provider and families/staff.
  • Document appropriate encounter and time for CCM and BHI services.
  • Schedule rosters should be updated daily.
  • Avoid scheduling changes without prior approval.
  • Prepare clinical documentation for assigned providers.
  • Perform medication reconciliation and enter lab results.
  • Host telehealth sessions as assigned.
  • Support facility staff with triage and discharge planning.
  • Communicate with the CCM team for enhanced patient care.

Qualifications

  • High School Diploma or equivalent.
  • Valid driver's license and reliable transportation.
  • Experience in medical scribing preferred.
  • Typing speed of 45 wpm or greater.
  • Clinician Support Specialist certification a plus.
