Clinician I - ACT
About the role
The Clinician I - ACT role is responsible for conducting and documenting individual and family psychosocial assessments; documenting individual crisis plans, creating individual treatment plans, documenting the findings of evaluations and progress in clients’ medical record; conducting and documenting family and/or couples treatment of clients and their families; maintaining contact with community resources for after care and providing comprehensive discharge planning services.
Responsibilities
- Conduct and document individual and family psychosocial assessments
- Document individual crisis plans, create individual treatment plans, document evaluation findings, and track client progress in medical records
- Conduct and document family and/or couples therapy sessions
- Maintain connections with community resources for post-care support
- Develop and implement comprehensive discharge plans
Requirements
- Master's Degree in Social Work/Clinical Counseling from an accredited institution
- Current applicable state(s) license as an LCSW (Licensed Clinical Social Worker), LMSW-CC (Licensed Master Social Worker, Conditional Clinical), LCPC (Licensed Clinical Professional Counselor), or LCPC-Conditional
- Valid Driver’s License if required for role
- Two years of relevant experience in a health or mental health setting preferred
