Clinician (FT) - Partial Care
Job Duties
- Bears primary responsibility for service coordination, provision or arrangement of services needed, personal advocacy, and development, review, and updating of individual treatment and service plans.
- Provides case management and support to PC clients; advocates for client needs; facilitates psycho-educational and skill building groups; develops individual recovery plans with clients;
- Functions as part of the PC Team; interfaces with the PC psychiatrist; maintains collateral contacts with other providers in the community on behalf of clients;
- Carries a caseload; and prepares documents in compliance with regulatory standards.
Requirements
- Master's degree in mental health or related field
- Valid LSW or LAC for Clinician I position
- Valid LCSW or LPC for Clinician II position
