Duties and Responsibilities

• Perform a comprehensive psychosocial assessment of assigned consumers.
• Complete crisis assessments, initial assessments and integrated summary forms.
• Develop and implement a plan of care with ongoing consumer input.
• Provide individual mental health and/or substance abuse treatment and crisis assessment.
• Perform record keeping in accordance with Health Information Services and other Center requirements.

Requirements

• Masters degree in Social Work, Human Services, or related field required.
• Candidate must have or be in the process of obtaining Independent license (LICSW and/or LMHC).
• Insured auto and valid driver's license.
