Schedule

• Per diem position with a minimum of 10 hours per week
• Fully remote position

Responsibilities

• Reviews and approves plan of care and evaluates proposed changes to the plan of care for clinical appropriateness
• Conducts regular OASIS reviews for all patients
• Ensures required visits are completed timely
• Reviews plans of care and other paperwork for accuracy and compliance with State and Federal requirements
• Monitors timeliness of required documentation to be uploaded to HCHB by direct care team members and LPN/LVN Clinical Coordinators, and maintains records and required documentation, assuring accuracy, completeness and compliance with licensing regulations, certifications standards and legal and ethical imperatives
• Participates in the organization's strategic planning and QAPI activities
• Performs other duties/projects as assigned
• Physically demanding, high stress environment

Requirements

• Current, unrestricted Connecticut Registered Nurse license
• Associate degree in Nursing
• Minimum of two years of RN experience in a home health or hospice setting
• Previous OASIS experience is required; candidates without OASIS experience cannot be considered
• Strong clinical documentation review and assessment skills
• Ability to work independently and manage deadlines in a remote environment
• Ability to quickly become proficient in Homecare Homebase, Workday, Microsoft Office, and other enterprise applications
• Availability to work a minimum of 10 hours per week and provide increased coverage as needed
• Ability to occasionally travel to support a field office during an audit or other business-critical event
• If travel is required, must have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state requirements
