About the role
Manages and supports an interdisciplinary team to provide hospice care to patients and families, ensuring compliance with regulatory standards and quality improvement initiatives. Demonstrates excellence in clinical practice and customer service, working under general supervision.
Responsibilities
- Assess medical, physical, social, emotional, and spiritual needs using an interdisciplinary approach.
- Provide support and guidance to families according to the care plan and policies.
- Participate in team meetings, committees, and quality initiatives to enhance care and compliance.
- Ensure regulatory adherence through audits and reviews.
- Lead quality improvement, educational activities, and clinical practice development.
- Manage daily department operations, including caseload, productivity, and fiscal targets.
- Oversee staff training, performance evaluation, hiring, and disciplinary actions.
- Support achievement of program goals and participate in special projects.
Requirements
- Current RN license in the state; Nurse Practitioner registration preferred.
- Bachelor’s degree in Nursing or related field.
- Minimum of three years of hospice clinical experience.
- Strong communication skills and clinical management experience preferred.
- Knowledge of computer applications is beneficial.
