Clinical Team Manager (RN)

About the role

Manages and supports interdisciplinary team members to deliver hospice care, including After Hours services, ensuring compliance with regulatory standards. Demonstrates excellence in clinical practice and customer service under general supervision.

Responsibilities

• Coordinates assessment of patients' medical, physical, social, emotional, and spiritual needs using an interdisciplinary approach.
• Provides support and guidance to patients' families in line with the Plan of Care and hospice policies.
• Participates in team meetings, committee work, and quality initiatives to improve care and processes.
• Ensures regulatory compliance through audits and monitoring of files and procedures.
• Contributes to quality improvement and educational activities, supporting professional practice standards.
• Supports leadership in achieving care goals and managing department operations, including caseload, productivity, and fiscal targets.
• Oversees staff training, performance evaluation, hiring, and other personnel decisions.
• Engages in special projects and performs managerial duties as needed.

Requirements

• Current RN license in the state; Nurse Practitioner license and DEA registration preferred.
• Bachelor's Degree in Nursing or related field required.
• At least three years of hospice clinical experience; management experience preferred.
• Effective communication skills; computer literacy in MS Office preferred.
