About the role

Manage and coordinate hospice care for patients and families, supporting an interdisciplinary team in providing comprehensive services, including After Hours care. Ensure compliance with regulatory standards and lead clinical and quality improvement initiatives. Demonstrate excellence in clinical practice and customer service under general supervision.

Responsibilities

  • Assess patients' medical, physical, social, emotional, and spiritual needs using an interdisciplinary approach.
  • Participate in team meetings, committees, and quality initiatives to enhance care and compliance.
  • Manage departmental operations, including caseload, productivity, budget, and staff performance.
  • Oversee staff training, performance evaluations, hiring, and disciplinary actions.
  • Support development and implementation of quality and educational programs.
  • Contribute to special projects and perform related duties as assigned.

Requirements

  • Registered Nurse license in New York required; Nurse Practitioner license and DEA preferred.
  • Bachelor’s Degree in Nursing or equivalent experience required.
  • Minimum of three years hospice clinical experience; clinical management experience preferred.
  • Effective communication skills and proficiency with MS Office are desired.
