Position Summary
The Clinical Support Specialist provides administrative and clinical coordination support to ensure the delivery of high-quality care to patients in home health and hospice settings. This role serves as a liaison between clinicians, patients, families, and office staff, supporting care planning, documentation, compliance, and day-to-day operations.
Reports to
Executive Director
Clinical Coordination
- Aid clinicians (RNs, LPNs, therapists, hospice staff) with care coordination and scheduling
- Support intake and referral processes, including gathering patient information and verifying eligibility
- Track and communicate patient care needs, updates, and changes in condition
- Coordinate interdisciplinary team (IDT) meetings and documentation
Documentation & Compliance
- Review clinical documentation for completeness, accuracy, and regulatory compliance
- Ensure timely submission of visit notes, plans of care, and physician orders
- Assist with electronic medical record (EMR) data entry, order tracking and maintenance
- Support compliance with CMS, DPH, CHAP regulations
Patient & Family Support
- Serve as a point of contact for patients and families regarding services and scheduling
- Provide basic education on home health or hospice services (non-clinical guidance)
- Facilitate communication between care teams, patients and families
Operational Support
- Manage clinician schedules and optimize visit routes
- Assist with supply coordination and equipment tracking
- Support quality improvement initiatives and audits
