Clinical Support Specialist
About the role
The Clinical Support Specialist role enables associates to work virtually full-time, except for required in-person training sessions, providing maximum flexibility and autonomy. This approach promotes productivity, supports work-life integration, and ensures essential face-to-face onboarding and skill development.
Responsibilities
- Assist Care Managers with administrative discharge planning activities across inpatient and outpatient settings.
- Monitor and track discharge planning milestones, barriers, and follow-up actions.
- Procures supplies and equipment in a timely manner to support patient independence in the home.
- Supports order, delivery, and receipt of Durable Medical Equipment (DME) and medical supplies as identified by patient care team.
- Facilitates care coordination with external medical providers and specialists.
- Identifies and schedules referrals with the interdisciplinary care team.
- Aids patients with scheduling visits with interdisciplinary care team members, as necessary.
- Affords non-clinical activities related to HEDIS expectations as determined by quality management leadership.
- Manages inbound and outbound inquiries and documents all correspondences within relevant systems timely.
Requirements
- Requires a H.S. diploma or equivalent and a minimum of 1 year experience or any combination of education and experience which would provide an equivalent background.
Preferred Skills, Capabilities, And Experiences
- Understanding of Medicaid/Medicare coverages of DME and supplies strongly preferred.
- Certified Nursing Assistant, Medical Assistant or Licensed Practical Nurse preferred.
