Clinical Support Specialist

About the role

The Clinical Support Specialist is responsible for providing day-to-day support to care team (including RNs, NPs, Pharmacists, and Social Workers) and providing care coordination for patients.

Responsibilities

• Assist Care Managers with administrative discharge planning activities across inpatient and outpatient settings.
• Monitor and track discharge planning milestones, barriers, and follow-up actions.
• Procures supplies and equipment in a timely manner to support patient independence in the home.
• Supports order, delivery, and receipt of Durable Medical Equipment (DME) and medical supplies as identified by patient care team.
• Facilitates care coordination with external medical providers and specialists. Identifies and schedules referrals with the interdisciplinary care team.
• Affords patients with scheduling visits with interdisciplinary care team members, as necessary.
• Affords non-clinical activities related to HEDIS expectations as determined by quality management leadership.
• Manages inbound and outbound inquiries and document all correspondences within relevant systems timely.

Requirements

• Requires a H.S. diploma or equivalent and a minimum of 1 year experience or any combination of education and experience which would provide an equivalent background.

Preferred Skills, Capabilities, and Experiences

• Understanding of Medicaid/Medicare coverages of DME and supplies strongly preferred.
• Certified Nursing Assistant, Medical Assistant or Licensed Practical Nurse preferred.

Qualifications

Unless specified as primarily virtual by the hiring manager, associates are required to work at an Elevance Health location at least once per week, and potentially several times per week. Specific requirements and expectations for time onsite will be discussed as part of the hiring process.
