Position Summary

The Clinical Support Specialist serves as a clinical resource and operational support partner for Marquis CHHA, providing expertise to ensure the delivery of high-quality, compliant, patient-centered home health services. This position supports clinicians and leadership through clinical oversight, workflow processing, quality assurance activities, staff education, documentation review, regulatory compliance monitoring, and process improvement initiatives.

Essential Duties And Responsibilities

  • Serve as a clinical resource to field clinicians regarding patient care, agency policies, documentation requirements, and best practices.
  • Aid in clinical case reviews to ensure patients receive appropriate, safe, and effective care.
  • Collaborate with interdisciplinary team members to promote coordination of care and positive patient outcomes.
  • Provide guidance regarding patient assessments, plan of care development, and appropriate service utilization.
  • Support clinicians in identifying and addressing complex patient needs.
  • Conduct reviews of clinical documentation for accuracy, completeness, reimbursement integrity, and regulatory compliance.
  • Maintain compliance with Medicare Conditions of Participation, New York State Department of Health regulations, and accrediting organization standards.
  • Assist with quality improvement initiatives and performance improvement projects.
  • Participate in audits, surveys, and corrective action plans as needed.
  • Identify documentation trends and recommend educational or process improvement interventions.
  • Aid in onboarding and orientation of new clinical staff.
  • Develop and provide ongoing education related to regulatory changes, clinical best practices, agency policies, and documentation standards.
  • Support competency assessment and skills validation activities.
  • Mentor clinicians to promote clinical excellence and professional development.
  • Support leadership in achieving agency quality, compliance, and performance goals.
  • Assist with workflow management and clinical operations as assigned.
  • Participate in case conferences, utilization review meetings, and interdisciplinary team meetings.
  • Assist with implementation of new programs, technologies, and operational initiatives.
  • Monitor and support timely completion of physician orders, care plans, and clinical documentation.
  • Provide direct patient care visits as needed based on agency operational needs and professional licensure.
  • Assist with patient care transitions and care coordination activities.
  • Support efforts to reduce hospitalizations, improve patient outcomes, and enhance patient satisfaction.

Qualifications

  • Education: Graduate of an accredited nursing, physical therapy, speech language therapy or occupational therapy program.
  • Licensure: Current and unrestricted New York State license as one of the following: Registered Nurse (RN), Physical Therapist (PT), Occupational Therapist (OT), Speech Language Pathologist (SLP).
  • Experience: Minimum of three (3) years of clinical experience in designated specialty; 1 or more years of experience in home health care preferred, but not required. Prior experience in quality assurance, clinical education, care management, utilization review, or supervisory roles preferred.
  • Knowledge, Skills, And Abilities: Thorough understanding of home health regulations, compliance requirements, and standards of practice. Strong clinical assessment and critical thinking skills. Excellent communication, coaching, and interpersonal skills. Ability to educate and mentor clinical staff. Strong organizational and time management abilities. Proficiency with electronic medical records (EMR) systems and Microsoft Office applications. Ability to work independently while managing multiple priorities.
  • Physical Requirements: Ability to sit, stand, walk, bend, and lift up to 25 pounds. Ability to use standard office equipment and technology. Ability to travel throughout the agency service area as needed.
