$26.00/hr. to $29.50/hr. (non-exempt), based on experience, qualifications, and location.
About the Role
As a Clinical Support Specialist, you will support clinical operations by managing essential electronic health record workflows, ensuring regulatory readiness, assisting with care coordination tasks, and providing administrative support that strengthens communication and efficiency across the Home Health team. Your work ensures clinicians have what they need to deliver safe, accurate, and meaningful care every day.
Responsibilities
- Manage and process client occurrences—including abnormal vital signs, fall reports, infection logs, and key physician communications—within the electronic health record.
- Place, process, and reconcile orders to support accurate, timely documentation.
- Monitor hospitalized and skilled nursing facility (SNF) patients, providing timely updates to clinical teams regarding discharge readiness or resume-care needs to support seamless continuity of care.
- Provide Clinical Team Admin Support by serving as the primary administrative partner to clinicians by assisting with EHR look-ups, supply ordering, and general workflow tasks that enhance efficiency and accuracy in care delivery.
- Enhance Communication & Coordination Across Teams by assisting with patient and facility inquiries, collaborating with internal departments, and supporting communication that promotes smooth operational flow and high-quality client experiences.
- Maintain Compliance & Technology Competency by staying current with HER functionality, organizational policies, and CMS requirements – supporting regulatory readiness through accurate processing of face-to-face documentation and other compliance tasks.
Requirements
- Licensed Practical Nurse (LPN) in State of Minnesota required.
- Prior experience in Home Health, Hospice, or healthcare administration strongly preferred.
- Proficiency with Homecare Homebase (HCHB) and/or other electronic health record systems.
- Excellent organization, prioritization, and follow-through skills.
- Strong verbal and written communication skills.
- Ability to solve problems and work efficiently in a fast-paced environment.
- Ability to pass state mandated background checks.
- Physical capability to perform essential job duties.
- Ability to read, write, and speak English to ensure effective communication.
