Work Setting: On-Site 5 days/week & hybrid opportunity after 6 months

Responsibilities

  • Assist the DON/ADONs in coordinating and overseeing all direct and indirect patient services provided by clinical organization personnel.
  • Provide guidance and counseling to clinicians to assist them in continually improving all aspects of home health care services, provided through organization personnel.
  • Assist DON/ADONs in managing clinical teams and planning.
  • Provide help in assessment, planning, implementation, and evaluation of patient and family/caregiver care to all clinical personnel as indicated.
  • Oversee daily scheduling, working closely with on-site scheduler.
  • Assist in interpreting operational indicators to detect census changes and increases or decreases in volume, which could impact staffing levels, revenues, or expenses.
  • Assist DON/ADONs to develop skills and techniques in evaluating the performance of clinicians.
  • Oversee the maintenance of patient clinical records, statistics, reports, and records for purposes of evaluation and reporting of organization activities.
  • Assure proper maintenance of clinical records in compliance with local, state, and federal laws.
  • Responsible for the maintenance of adequate and appropriate inventory supplies and equipment for the provision of patient services.
  • Plan and implement in-service and continuing education programs to meet education and training needs of organization personnel.
  • Assist with the evaluation of organization performance via performance improvement program, productivity, quarterly, and annual reviews.
  • Assure the quality and safe delivery of home health services provided through the Organization.
  • Assist in the development of organization goals.
  • Develop, recommend, and administer Organization policies and procedures.
  • Stay informed about changes in the field of nursing and home health care; share information with appropriate organization personnel.
  • Promote home health referrals in the health care community.
  • Other duties assigned.

Qualifications

  • Active LPN License.
  • Ability to work independently or as an active member of a team.
  • Ability to multitask, prioritize, delegate, and handle constant changes in workflow.
  • Goal-oriented and organized team player, with strong interpersonal skills and ability to work effectively at all levels in a collaborative team environment.
  • Highly developed observation, verbal and written communication skills, and problem-solving skills.
  • Thorough knowledge and understanding of clinical protocols, procedures, and standards of home health care and AL/IL facilities.
  • Ability to provide operational guidance and leadership to staff.
  • Ability to plan, implement, and evaluate individual patient care programs.

Pay

$26.00 - $29.00/hour

Schedule

Monday - Friday; standard business hours (40 hours/week)

Pre-Employment Requirements

  • Must successfully pass a fingerprint-based background check in accordance with state and federal regulations.
