The Patient Care Manager Assistant supports the Patient Care Manager in coordinating staff to deliver high-quality, efficient care aligned with physician orders and regulatory guidelines.
Responsibilities
- Preparing patient lists
- Participating in weekly case conferences
- Providing direct patient care within scope
- Assisting with clinician documentation reviews to ensure accuracy and completeness
Qualifications
- Current unrestricted LPN license in Washington
- Valid driver's license, vehicle insurance, and reliable transportation
