CLINICAL SUPERVISOR & CARE COORDINATOR

Full-time position (40 hours/week), Monday–Friday, 8:00 AM–5:00 PM. No weekends, holidays, or on-call shifts.

About the role

The Clinical Supervisor/Care Coordinator RN provides quality triage and patient care using strong assessment skills and point-of-care testing within the scope of practice defined by the State Board of Nursing. In collaboration with the Practice Manager and clinicians, this role ensures the delivery of high-quality care, develops and maintains mechanisms to evaluate and improve care quality, enhances care coordination, and boosts patient and customer satisfaction. The position fosters a collaborative environment among clinical staff and other disciplines, working closely with clinicians to improve quality, patient satisfaction, and the ongoing development of the practice and staff.

Responsibilities

• Provide direct supervision of clinical staff.
• Deliver comprehensive nursing care in high-volume Cardiology and Neurology clinics, performing detailed assessments and triaging patients with acute and chronic cardiac conditions.
• Manage patients with cardiac implantable electronic devices (CIEDs), including pacemakers, ICDs, and CRT devices.
• Conduct in-clinic and remote device interrogations, evaluating battery status, lead integrity, and arrhythmia events; collaborate with providers to optimize device programming.
• Assist with pre- and post-procedure care for device implantation, including patient preparation, wound care assessment, and complication monitoring.
• Educate patients and families on cardiovascular/neurology disease management, medications, device function, activity restrictions, and lifestyle modifications to improve outcomes.
• Ensure adherence to clinical protocols, safety standards, and regulatory requirements in both in-person and remote monitoring workflows.
• Review and coordinate care for patients in the CHF program identified as high risk and/or in need of care coordination with community resources to promote efficiency and proactive care.
• Act as a liaison between patients, families, and healthcare providers, managing patient care plans, scheduling, and resources to ensure high-quality, efficient care.

Requirements

• Graduate of an RN-accredited nursing program; BSN preferred.
• 3–5 years of recent outpatient clinical nursing experience preferred.
• Acute care experience preferred.
• Previous experience with cardiac implantable electronic devices preferred.
• Active State of Vermont RN license.
• Current Healthcare Provider CPR certification.

• Ability to read and write, with hand-eye coordination and manual/fine dexterity.
• Ability to distinguish letters, numbers, or symbols accurately.
• Use of office equipment such as computers, telephones, copiers, and fax machines.
• Corrected hearing and vision to normal range; absence of color blindness.
• Ability to work under stressful conditions (high office volume, high level of phone work) and irregular hours.
• Exposure to communicable diseases or body fluids; potential for repetitive strain injuries and interruptions.
• OSHA Category I – tasks involve exposure to blood, body fluids, or tissues, requiring adherence to Universal Precautions and bloodborne pathogen standards.
• On-the-job time spent standing, walking, sitting, talking, or hearing.
• Ability to lift or exert force up to 30 pounds.
• Vision requirements: close vision, color vision, and ability to adjust focus.
• Ability to hear calls and instructions from physicians, department staff, and others.
• Typical noise level: moderate in an office environment.

Pay

Salary range: $83,200.00 – $126,859.00. Final compensation will be determined based on relevant experience, education, internal equity, and market competitiveness.
