In this dynamic leadership role, you’ll help residents thrive by supporting their daily care needs and creating a warm, welcoming culture of home within each Household. You’ll also train, guide, and supervise our dedicated Resident Assistants, Med Techs, and LPNs, empowering them to deliver exceptional, person-centered care. Through compassionate leadership, you’ll encourage residents’ resilience and help them live with purpose, connection, and confidence—even while navigating significant life challenges. What You'll Do Ensures residents’ privacy, respect, dignity and rights are protected as per State and Federal Regulations. Maintains a positive attitude as well as standards of good nursing practice. Maintains assessments of residents’ pre-admission, 72 hours after admission, upon significant changes, and including post-hospitalization. Maintains an active role in resident care plans to assure all therapeutic aspects have been addressed; assesses residents’ levels of care needs and ensures proper notification to a resident's family/responsible party. Plans, organizes and directs effective supervision of each Meadow View neighborhood to make sure residents thrive in a comfortable, clean and safe environment. Coordinates resident care services with other disciplines such as Activities and Dining Services. Provides general supervision, guidance and assistance for a resident in implementing his/her personal health program, to assure that preventative measures, treatments, diet and other health services prescribed are properly carried out and recorded. Assists with the documentation of and ensures compliance with all department policies and procedures. Oversees all medication changes and medication passes to ensure staff compliance, and compliance with State Regulations. Performs medication passes on occasion or as needed, and covers shifts when needed. Assists residents in the transition from Meadow View to Skilled Nursing Care. Ensures accurate documentation of all medical records and reporting forms. Develops and maintains a written care plan (RASP) for Meadow View residents. Coordinates annual Medical Evaluations (and MA51s when necessary), along with annual physician appointments. Completes all annual screens on all residents. Perform Med Tech, Resident Assistant, Activities, Homemaker, and Housekeeping functions as needed. Informs the Director of Meadow View Personal Care of a residents’ changing conditions, as well as other pertinent happenings on the unit. Responsible to assess supply and equipment needs for the department, and oversees that units are adequately stocked and maintained; ensures that staff is properly charging residents for supplies used and additional services provided. Performs daily rounds on Meadow View units to ensure that policies are being followed and residents are being cared for properly. Assists with resident admissions, transfers, discharges and deaths. Coordinates with the RN on call to call regarding resident deaths as needed. Conducts or coordinates the pharmacy medication administration course for team members; ensures that new LPNs and Med Techs are properly oriented to their positions, and ensures that they obtain the knowledge appropriate to perform their duties. Trains all team members on medical equipment including proper use and upkeep. Ensures staff is compliant with documentation requirements including but not limited to MAR's, ADL flow sheets and clinical record documentation; receives and processes physician orders including faxed and written orders. Meets with all visiting doctors regarding resident treatment and/or evaluation; gathers supporting documentation to provide to all applicable doctors; reviews and implements new orders as required. Provides supervisory duties in the absence of the Director of Meadow View Personal Care. Assists in 90 day introductory, coaching and teachable moments evaluation processes. Assists in the discipline process of LPNs, Resident Assistants and Med Techs. Ensures that the facility operates in compliance with DHS Regulations. Answers all incoming calls in a timely and courteous fashion. Attends department meetings, staff meetings and training programs. Participates in the emergency call rotation. Participates in monthly fire drill procedures and assists in the prompt evacuation of residents. Oversees the Med Tech Trainer Program. Acts as the contact person to communicate all resident medical changes. Preforms other duties and responsibilities as assigned What We're Looking For Registered by the Commonwealth of Pennsylvania as a Licensed Registered Nurse or Licensed Practical Nurse. Possesses initiative, judgment, dependability, and leadership skills. Possesses the ability to work cooperatively with others and to delegate wisely.
