Responsibilities
- Provides strong leadership and direction of all case management and clinical support services with a trauma-informed and housing-focused clinical approach.
- Facilitates bi-weekly clinical case conferencing with case managers and housing staff to provide updates on client participation, housing status, behavioral health concerns, crisis intervention needs, and barriers to housing stability.
- Conducts monthly reviews of all active client case files to ensure documentation is accurate, timely, compliant, and reflective of service goals and housing plans.
- Reviews HMIS and other required databases weekly to ensure all data is entered accurately and within established timelines.
- Conducts routine face-to-face supervision of assigned staff no less than once per month and provides ongoing coaching, performance feedback, and professional development support.
- Safeguards appropriate documentation on clients including assessments, service plans, progress notes, referrals, housing plans, discharge summaries, incident reports, and daily logs.
- Supports crisis intervention efforts and assists staff with de-escalation, safety planning, and behavioral health coordination as needed.
- Develops and supports MSW student internship opportunities, including provision of clinical supervision, field instruction, and staff mentorship.
- Provides clinical supervision for staff seeking Maryland social work licensure in accordance with Maryland Board of Social Work Examiners requirements.
- Collaborates with community behavioral health providers, healthcare organizations, landlords, and external partners to strengthen service coordination and continuity of care.
- Aids in the implementation and monitoring of clinical quality improvement initiatives, accreditation standards, and agency best practices.
- Supports staff wellness and promotes a culture of accountability, professionalism, equity, and trauma-responsive care.
Qualifications
- Masters in Social Work (MSW)
- Valid Maryland Social Work License
- Experience in case management and clinical supervision
- Knowledge of Trauma-Informed Care, Housing First, Harm Reduction, Crisis Prevention, and client-centered practice
- Experience with HMIS and other required databases
- Strong leadership and supervisory skills
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
