About the role
This onsite position places you directly in the heart of client care, where your compassion, clinical insight, and problem-solving abilities will help transform lives every day.
Responsibilities
- Participate in daily meetings, case discussions, and treatment planning with a highly collaborative PACT team.
- Provide both individual and group counseling using evidence-based practices.
- Conduct emergency mental health assessments.
- Closely coordinate with medical staff regarding client medication needs.
- Support families and natural support systems, helping them understand and engage in the treatment process.
- Ongoing assessment, coordination with community partners, and assisting with transportation needs to help clients move toward greater independence.
- Contribute to the team's on-call rotation.
- Engage in continuous quality improvement efforts.
Requirements
- A Master's degree in social work, psychology, or another human services field.
- At least two years of post-bachelor's experience working with individuals with severe and persistent mental illness.
- Strong knowledge of community resources and support agencies.
- A valid New Jersey driver's license with a clean five-year driving record.
- Spanish-speaking ability strongly preferred.
