Clinical Social Worker - MSW - Integrated Behavioral Medicine

We’re looking for a Clinical Social Worker to collaborate with Primary Care Providers and a Collaborating Psychiatrist to deliver comprehensive behavioral-health treatment and care management.

Responsibilities

• Perform psychosocial assessments of patients and families to identify emotional, social, and environmental strengths and problems related to diagnosis, illness, treatment, and life situation.
• Collaborate with the multidisciplinary team to provide mental-health support for a designated caseload.
• Monitor patient progress using validated scales and clinical judgment; intervene as needed to ensure the plan of care is patient-focused, high-quality, efficient, and cost-effective.
• Provide brief interventions, psychoeducation, and direct care management around issues tied to diagnosis, illness, treatment, and life situation.
• Assess needs with the patient and/or family, develop an individualized care plan, and arrange resources in collaboration with the care team.
• Formulate, develop, and implement a comprehensive psychosocial treatment plan using appropriate clinical social-work treatments and interventions (e.g., crisis intervention, brief individual, marital, family, or group therapies, grief and bereavement work).
• Assist the multidisciplinary team in screening, identifying, diagnosing, managing, and treating victims of abuse, neglect, domestic violence, rape, etc.
• Provide consultative services to health-care team members within scope-of-care definitions.
• Maintain working knowledge of relevant medical/legal issues (e.g., advance directives, child and elder abuse).
• Work closely with the Primary Care Provider and Collaborating Psychiatrist to diagnose, treat, and manage mental-health and/or substance-abuse problems in patients and family members.
• Document assessments, plans, interactions, and interventions according to departmental, hospital, and health-system guidelines.
• Access and apply knowledge of hospital, medical-center, and health-system resources to meet patient-care needs.
• Stay current with social-work and health-care developments; seek further enhancements of job-related knowledge.
• Provide teaching and training for students, staff, and faculty from social work and other disciplines.
• Contribute to the academic mission through participation in committees, conferences, publications, teaching, and research.
• Exhibit consistent professional behavior; attend and participate in staff, committee, department, and other administrative meetings.

Requirements

• Master’s degree in Social Work or related field.
• Current state license as required by the board where services will be provided:
  • MD: Licensed Master Social Worker (LMSW), Licensed Certified Social Worker (LCSW), or Licensed Certified Social Worker–Clinical (LCSW-C) through the Maryland Board of Social Work.
  • OH: Licensed Social Worker (LSW) or Licensed Independent Social Worker (LISW) through the Ohio Board of Social Work.
  • PA: Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) through the Pennsylvania Board of Social Workers, Marriage and Family Therapists and Professional Counselors.
  • WV: Licensed Graduate Social Worker (LGSW), Licensed Certified Social Worker (LCSW), or Licensed Independent Clinical Social Worker (LICSW) through the West Virginia Board of Social Work.
• Two years of direct clinical experience with behavioral health.

Preferred Qualifications

• Current state license at the highest clinical level where services will be provided:
  • WV: Licensed Certified Social Worker (LCSW) or Licensed Independent Clinical Social Worker (LICSW).
  • MD: Licensed Certified Social Worker (LCSW) or Licensed Certified Social Worker–Clinical (LCSW-C).

Schedule

40 hours per week, exempt position.
