Part-time position working variable days, Monday to Friday.
Scope of Work
To work collaboratively with members of an interdisciplinary team to assist patients and families with complex psychosocial needs. These interventions may include:
- Alternate site coordination
- Discharge planning
- Addressing all forms of identified abuse/neglect
- Adoption support
- Substance use disorder interventions
- Assisting with end-of-life decisions
- Providing appropriate referral resources
- Bereavement support
- Adjustment counseling
- Facilitating guardianships
- Behavioral health concerns, including psychiatric transfers
Qualifications
- Master’s degree in social work Required
- 2 years of relevant experience Required
- Health care related experience post obtaining master’s degree in social work Preferred
- Care Management experience Preferred
- Individual, family assessment, crisis interventions, grief and loss counseling, discharge planning experience Preferred
- Fully Licensed-Master Social Worker (LMSW-Master) - State of Michigan Required
