We’re looking for a Clinical Social Worker to collaborate with Primary Care Providers and a Collaborating Psychiatrist, delivering comprehensive treatment and care management for behavioral health.

Responsibilities

  • Provides psychosocial assessments of patients and families to identify emotional, social, and environmental strengths and problems related to diagnosis, illness, treatment, and life situation.
  • Collaborates with all members of the multidisciplinary team to provide comprehensive mental-health support for a designated caseload.
  • Monitors patient progress using validated scales and clinical judgment; intervenes as necessary to ensure the plan of care is patient-focused, high-quality, efficient, and cost-effective.
  • Provides brief interventions, psychoeducation, and direct care management to patients and families around issues related to diagnosis, illness, treatment, and life situation.
  • Meets with patients and/or families to assess needs, develop individualized care plans, and arrange resources in collaboration with the care team.
  • Formulates, develops, and implements a comprehensive psychosocial treatment plan using appropriate clinical social-work treatments and interventions (e.g., crisis intervention, brief individual, marital, family, or group therapies, grief and bereavement work).
  • Assists the multidisciplinary team in screening, identifying, diagnosing, managing, and treating victims of abuse, neglect, domestic violence, rape, etc.
  • Provides consultative services to healthcare team members within scope-of-care definitions.
  • Maintains working knowledge of relevant medical/legal issues (e.g., advance directives, child and elder abuse).
  • Works closely with Primary Care Providers and Collaborating Psychiatrists to diagnose, treat, and manage mental-health and/or substance-abuse problems in patients and family members.
  • Documents assessments, plans, interactions, and interventions according to departmental, hospital, and/or health-system guidelines.
  • Accesses and provides hospital, medical-center, and health-system resources to meet patient-care needs.
  • Maintains current knowledge of therapeutic techniques, interventions, and available resources; stays updated on social-work and healthcare developments.
  • Provides teaching and training for students, staff, and faculty from social work and other disciplines.
  • Contributes to the academic mission through participation in committees, conferences, publications, teaching, and research.
  • Exhibits consistent professional behavior; attends and participates in staff, committee, department, and administrative meetings.

Requirements

  • Master’s degree in Social Work or related field.
  • Current state license as required by the board where services will be provided:
    • Maryland: LMSW, LCSW, or LCSW-C
    • Ohio: LSW or LISW
    • Pennsylvania: LSW or LCSW
    • West Virginia: LGSW, LCSW, or LICSW
  • Two years of direct clinical experience in behavioral health.

Preferred Qualifications

  • Current state license at the highest clinical level where services will be provided:
    • West Virginia: LCSW or LICSW
    • Maryland: LCSW or LCSW-C

Schedule

Full-time, 40 hours per week.
