Position Summary
Key Responsibilities
Performing psychosocial assessments and developing interventions and treatment plans in collaboration with the multidisciplinary CAD team during outpatient clinics.
Providing social work support to patients and families who are managing new and/or chronic conditions, addressing the emotional impact and connecting families to resources through the hospital or in their communities as needed.
Providing supportive documentation (such as school letters, letters of medical necessity, etc.) in collaboration with the team, as needed.
Education
- Minimum Qualifications: A master's degree in social work is required.
- Experience: Minimum of two plus years of previous social work experience required; exposure within a medical setting preferred.
- Flexibility and ability to work with a multidisciplinary team, in a fast-paced, dynamic environment.
- Excellent communication and collaboration skills are expected.
- Licensure/Certifications: LICSW licensure required, in the state of Massachusetts.
