About The Role
Join our team at Progress West Hospital to make a direct impact on patient care and operations. Primary focus: supporting the ED to help reduce readmissions and revisits, and assisting with inpatient social work for the current inpatient social worker.
Full Time - 40 Hours Per Week, Monday-Friday, rotating weekends on call. Day Shift: 8:00 AM - 4:30 PM. Benefits eligible.
Minimum Requirement: MSW and LCSW license.
Overview
Barnes-Jewish St. Peters Hospital proudly serves the health care needs of families in St. Charles, Lincoln, and Warren counties. The 102-bed community hospital provides comprehensive inpatient and outpatient services including the Siteman Cancer Center, the Breast Health and Women’s Center, the Cardiology Center, the Neurosurgery Center, and the Outpatient Surgery and Endoscopy Center. Barnes-Jewish St. Peters Hospital is an Accredited Chest Pain Center and carries a Level II stroke certification. Additionally, the hospital’s medical office building houses offices for specialists in primary care, cardiology, obstetrics, dermatology, surgery, and pulmonology. Outpatient services include physical therapy, a sleep lab, and cardio-pulmonary rehabilitation.
At Barnes-Jewish St. Peters Hospital, every decision is made in the best interest of patients and families. The Patient Care Team is responsible for assessing, planning, implementing, and evaluating each patient's plan of care to facilitate a clinically safe transition home.
Responsibilities
- Conducts screening and assessment activities to meet the needs of the discharged patient.
- Applies assessment findings to prepare a patient-centered plan of care.
- Applies and complies with all regulations and utilizes resources available to patients to manage their continuum of care.
- Provides individual and group counseling, linkage to community resources, expertise in cultural diversity, and advocacy.
- Collaborates with the health care team and ensures timely, cost-effective discharge and transition planning.
Requirements
- Master’s Degree in Social Work
- Licensed Clinical Social Worker (LCSW)
- No prior experience required; 2-5 years preferred
