Additional Information About The Role

Primary Focus: supporting the ED at Progress West Hospital to help reduce readmissions and revisits, and also assisting with inpatient social work assistance for the current inpatient social worker.

Full Time - 40 Hours Per Week, M-F, Rotating Weekends on call

Day Shift: 8am-4:30pm

Location: Progress West Hospital

Overview

Barnes-Jewish St. Peters Hospital proudly serves the health care needs of families in St. Charles, Lincoln and Warren counties. The 102-bed community hospital provides comprehensive inpatient and outpatient services including the Siteman Cancer Center at Barnes-Jewish St. Peters Hospital, the Breast Health and Women's Center, the Cardiology Center, the Neurosurgery Center and the Outpatient Surgery and Endoscopy Center. Barnes-Jewish St. Peters Hospital is an Accredited Chest Pain Center and carries a Level II stroke certification. Additionally, the hospital's medical office building houses offices for specialists in primary care, cardiology, obstetrics, dermatology, surgery and pulmonology. Barnes-Jewish St. Peters Hospital also offers outpatient services including physical therapy, a sleep lab, and cardio-pulmonary rehabilitation. At Barnes-Jewish St. Peters Hospital, every decision is made in the best interest of patients and families. The Patient Care Team is responsible for accesses, plans, implements, and evaluates each patient's plan of care to facilitate a clinically safe transition home.

Preferred Qualifications

Role Purpose: Provides clinical social work services to address the psychosocial needs of patients and families. Provides individual and group counseling, linkage to community resources, expertise in cultural diversity and advocacy. Collaborates with the health care team and ensures timely, cost-effective discharge and transition planning.

Responsibilities

  • Conducts screening and assessment activities to meet the needs of the discharged patient.
  • Applies assessment findings to prepare a patient-centered plan of care.
  • Applies and complies with all regulations and utilizes resources available to patients to manage their continuum of care.

Minimum Requirements

  • Education: Master's Degree Social Work
  • Experience: No Experience
  • Supervisor Experience: No Experience
  • Licenses & Certifications: Lic Clinical Social Worker

Experience Preferred

  • 2-5 years
