Clinical Social Worker LCSW
About the role
As our Social Worker, you will be a compassionate advocate, providing vital support and guidance to individuals and families facing complex challenges. You will conduct psychosocial assessments, develop care plans, provide counseling and crisis intervention, and connect clients to resources. You'll advocate for clients and collaborate with multidisciplinary teams to ensure holistic support and promote well-being.
Responsibilities
- Conduct psychosocial assessments and develop care plans
- Provide counseling and crisis intervention
- Connect clients to community resources
- Advocate for clients within multidisciplinary teams
- Meet discipline-specific productivity standards
- Demonstrate competency in social work with hospice patients/families during field observation and testing
- Complete and submit documentation following established guidelines
- Ensure physician orders are written according to policy
- Document follow-up of identified problems
- Review and document clinical progress toward goals
Requirements
- Current California LCSW license
- 1-3 years of experience
- 2 years in a healthcare setting
- Basic Life Support (CPR) certification, upon hire
- Valid California Driver’s License, upon hire
- Auto Insurance, upon hire

Pay
$62.33 - $70.52 per hour
