About the Role
We are seeking a dynamic individual with strong clinical, collaborative, and consultation skills to join our Transplant Social Work team.
Responsibilities
- Support the Kidney/Pancreas Transplant Program by conducting comprehensive psychosocial assessments to determine candidacy for kidney and/or pancreas transplantation and presenting your recommendations to the multidisciplinary Selection Committee.
- Provide ongoing support and care coordination to patients on the transplant wait list as they prepare themselves and their support systems for transplantation.
- Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team.
- Monitor, evaluate, and record client progress according to measurable goals described in the treatment and care plan.
Qualifications
- Education: Master's Degree in Social Work required
- Experience: Unlicensed: Grace Period for License Eligibility [LCSW -Massachusetts] Social Worker I Clinical experience in medical setting 0-1 year required
Licensed Clinical Social Worker [LCSW - State License] required
Social Worker II Clinical Experience in a medical setting 2-3 years required
Licensed Independent Clinical Social Worker [LICSW - State License] required
