Responsibilities

  • Provides psychosocial assessment, crisis intervention and supportive services for oncology patients, families and staff.
  • Is an active member of the multidisciplinary team contributing to team goal setting and outcomes.
  • Identifies inequities in patient experience and care; advocates to help ensure equity in health care; and in connection to applicable community resources.
  • Continually evaluates own work and practice to ensure equity and inclusivity in patient and team interactions, clinical and general documentation, etc.
  • Develops and implements treatment plans/community linkages related to psychosocial needs and adjustment to illness in collaboration with other team members.
  • Provides education to patients, families, staff and community related to psychosocial needs and interventions.
  • Assesses, intervenes and reports intimate partner violence/relationship violence, child/elder abuse and other reportable situations in accordance with legal mandates.
  • Provides support and acts as a resource for complex Durable Power of Attorney and other legal issues (such as End Of Life Option Act for those who opt-in).
  • May facilitate patient and family groups.
  • Attends department, hospital and community in-services related to field of practice.
  • Participates in quality improvement activities and program planning for the Department, Hospital and Service Lines.
  • Collaborates effectively with other departments such as the Cancer Resource Center (Patient and Family Cancer Support Center), Oncology Clinics, Psycho-Oncology, etc.
  • Assesses and intervenes with appropriate advocacy, education and referrals related to access to health care, including health coverage and benefits counseling, referrals for MediCal, and advocacy within the public and private sectors of insurance and disability benefits.

Qualifications

  • Required for Clinical Social Worker II: Master’s degree from a CSWE accredited school of Social Work. Minimum of three years experience supporting individuals with complex health conditions, preferably with cancer patients. Knowledge and abilities essential to the successful performance of the duties assigned to the position. Demonstrated clinical experience utilizing skills in individual, family and group interventions. Knowledge of issues of access to health care and disability benefits. Demonstrates resourcefulness, effective written and oral communication, diplomacy, organizational and analytic skills. Self-directed, assertive and creative in problem solving, systems planning and patient care management in a high volume and complex work environment. Basic computer skills. Ability to work effectively and collaboratively with interdisciplinary teams.
  • Required for Clinical Social Worker III: Licensed in Clinical Social Work in the state of California/Board of Behavioral Sciences. Advanced clinical knowledge and skills and is distinguished from the CSW II by leadership activities including participation in advanced program planning, provision of educational programs, clinical supervision, consultation to the clinical team, other departments, and the community.
