About the role

The Clinical Social Worker intervenes with patients who have complex psychosocial needs, requires assistance with eligibility determination for social programs and funding sources, and coordinates community assistance from various special funds and agencies. They offer crisis intervention to pre/post-Kidney Transplant patients and families with psychosocial needs, and develop discharge plans for high-risk patient populations. The CSW coordinates resources and services to meet patient psychosocial needs while collaborating with Case Managers, Nursing Staff, Medical Staff, and ancillary departments.

Responsibilities

  • Provide psychosocial assessments and interventions based on preliminary risk screening, evaluating patient and family psychosocial risk factors.
  • Intervene with patients and families regarding emotional, social, and financial consequences of illness or disability; collaborate with CSW III to manage complex family dynamics impacting patient care and discharge.
  • Manage clinical resources and transition planning for patient caseload from admission through discharge, identifying and addressing barriers to timely discharge.
  • Facilitate care coordination with multidisciplinary treatment teams for optimal patient outcomes and timely discharge when medically ready.
  • Collaborate with Psychiatry and other appropriate teams on cases with psychiatric holds or primary mental health/substance abuse problems, and address domestic violence, child/elder abuse, and other reportable situations.
  • Educate patients and families on post-acute options and address treatment decisions and end-of-life issues; identify surrogate decision makers and facilitate completion of Advance Health Care Directives and POLST.
  • Comply with Department standards for timely submission, accuracy, and completeness of documentation in the Medical Record and collect required statistical data.
  • Attend department, hospital, and community in-services related to field of practice and participate in quality improvement activities and program planning.
  • Provide clinical supervision to CSW II staff for LCSW licensure compliance.
  • Collaborate and communicate with the multidisciplinary transplant team on evaluation, listing, and management of pre- and post-transplant patients, developing materials and providing education.
  • Facilitate transplant support groups and education seminars as appropriate.

Qualifications

  • Masters degree from a CSWE accredited school of Social Work.
  • Experience (internship/career) in a healthcare and/or hospital setting preferred.
  • Knowledge and abilities essential to the successful performance of the duties assigned to the position, including knowledge of relevant community resources commensurate with illness, symptoms, mental status, and impairments in functioning and knowledge of the medical and psychological sequelae of course of illness and impact on social functioning (may be specific to patient population).
  • Demonstrates resourcefulness, effective written and oral communication, diplomacy, and organizational and analytic skills.
  • Self-directed, assertive, and creative in problem-solving, systems planning, and patient care management in a high-volume work environment.
  • Ability to work effectively and collaboratively with interdisciplinary teams.
  • Basic pager, telephone, and computer skills including the use of the internet.
  • Licensed in Clinical Social Work in the state of California/Board of Behavioral Sciences.
  • Three years of experience in a healthcare setting required and hospital setting preferred.
  • Preferred: Current California State License of Clinical Social Work in good standing.
