Clinical Social Worker I PRN

Duties & Responsibilities

• Develops, implements, and monitors service/treatment interventions that provide a variety of services to meet patient/family needs.
• Provides emotional support to multidisciplinary team members in the context of patient care.
• Participate in on call and coverage of other CSW team members.
• May serve on department and hospital-wide committees as directed.
• Develops a therapeutic relationship with the patients and families to assist with the social, financial, and psychological problems associated with hospitalization and/or outpatient treatment.
• Develops and maintains relationships with peers within and outside of the department as well as community resources.
• Conducts initial needs assessment to gather information on the patient/family’s medical, social, and financial history.
• Serves as a liaison with multi-disciplinary care teams and external community agencies to identify, implement, and monitor treatment goals.
• Facilitates staffing, care conferences, etc. to coordinate care between the treatment team.
• Compiles and prepares case records, reports, and documents that comply with state and federal standards in providing case notes, treatment plans, and evaluations.
• May provide clinical supervision and support to clinical social work interns.

Minimum Qualifications

• Degrees: Masters in Social Work
• Area of Study: Social Worker
• Experience: Minimum Experience Required: One (1) year of post-Master’s full-time work experience providing psycho-social assessments and interventions to patients and families
• Licenses & Certifications: Licensed Clinic Social Worker

Salary Information

Pay is dependent on applicant's relevant experience. Hourly Range: $35.17 to $52.76
