Responsibilities
- Assesses the psychological, social, emotional, cultural, or financial needs that impact patient health and recovery.
- Provides professional social work services utilizing supportive counseling and clinical knowledge in assessing the needs of patients and families during illness, crisis intervention, and post-discharge planning.
- Collaborates with members of the healthcare team in the development of a transition plan and addresses needs in a timely manner.
- Participates in program planning and development to meet the needs of customers within the healthcare system and community.
- Completes family therapy sessions with patients and their families.
- Participates in daily treatment team meetings.
- Provides psychoeducational group therapy sessions to patients.
- Affords discharging planning, makes referrals, and assists with transportation arrangements.
Requirements
- Master's in Social Work required.
- Licensed Masters Social Worker (MO) - Missouri Division of Professional Registration required.
Job Requirements
- Applicable Experience: Less than 1 year.
- Licensed Masters Social Worker (MO) - Missouri Division of Professional Registration.
- Master's Degree.
