Function as an independent practitioner, work under stressful conditions, demonstrate assertiveness skills, work well with others, perform quality assessments, have knowledge of family systems, demonstrate effective communication techniques, be flexible in job responsibilities, provide thorough clinical assessments using a biopsychosocial model, communicate patient's situation and problems, provide professional social work assistance to patients and families, make independent assessments, utilize information available, document assessments according to department guidelines, consult with LCSW on cases with a safety component, devise and implement appropriate therapeutic interventions and plans, document continued reassessment of provision of social work services, transitions cases appropriately, update documentation when plan changes, maintain a 70% direct patient contact ratio, establish rapport with the patient and family, provide continuity of care regardless of shift parameters, actively participate in problem solving strategies related to patient care, seek opportunities to improve positive social determinants of care within and outside the Social Work Department.
