Clinical Social Worker - Emergency Department - Rotating

About the role

The Emergency Department Clinical Social Worker 2 delivers discharge planning and social work services during normal business hours and the entire hospital after hours. This job requires interaction with a variety of patient populations including, but not limited to, neonatal, pediatric, adolescent, adult and geriatric. This job provides direct intervention with patients and families and jointly with the health care team, provides assessment, problem solving, advocacy, community referrals and crisis intervention services. Responsible for the daily management of all complex discharge planning and social work functions.

Responsibilities

• Provide direct intervention with patients and families
• Conduct assessment, problem solving, advocacy, community referrals, and crisis intervention
• Manage daily complex discharge planning and social work functions
• Develop, implement, and continually revise interdisciplinary plans of care for Emergency Department patients
• Negotiate and collaborate with the interdisciplinary team, community resources, and payers to facilitate timely and cost‑effective discharge plans that enhance health and social services across the continuum of care

Qualifications

• Licensed Clinical Social Worker in the State of Virginia (preferred)
• Minimum of one (1) year of clinical social work experience (required)
• Preferred experience in acute care settings, acute discharge planning, home health, case management, long‑term care, or related community agencies
• Master of Social Work degree from a CSWE‑accredited program (required)
• Serves all age groups (neonatal, pediatric, adolescent, adult, geriatric)
• Physical: ability to lift less than 20 lbs.; prolonged sitting; walking (distance)
• Mental/Sensory: strong recall, reasoning, problem solving, hearing, clear speech, legible writing, reading, logical thinking
• Emotional: ability to thrive in a fast‑paced environment, handle multiple priorities, engage in frequent and intense customer interactions, and adapt to frequent change

Pay

Compensation range for ED Clinical Social Worker 2: $28.22 – $42.28 per hour (actual salary based on relevant work experience, credentials, and qualifications).

Schedule

Day, evening, night, and weekend shifts are available.
