JOB REQUIREMENTS: KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED
- Master's degree in social work from an accredited school.
- Minimum 2- years of experience in health care, with special interest and understanding of mental health diagnosis, substance issues, and interpersonal/domestic abuse.
- LCSW licensure with plan for LICSW.
- Excellent interpersonal and organizational skills. Strong computer skills. Demonstrated ability to work collaboratively in interdisciplinary teams.
- Ability to exercise sound clinical judgement in fast-paced and demanding settings.
The Clinical Social Worker (CSW)
- Responsible for assisting patients and their families or significant others with the stress and difficulties related to illness, personal issues, financial difficulties, and other emotional/mental health issues.
- Interventions consider the patient’s strengths, resources, insurance coverage, family system, and motivation for change.
- Collaborates with nurse care managers and other members of the multidisciplinary team to facilitate safe and effective discharge plans.
- Supports the Hospital’s mission and participates in performance improvement and patient safety initiatives.
- Must have a passion for working with elders who may be more vulnerable due to advanced age or medical comorbidities.
- Develops trusting relationships with diverse patients and their families.
Duties and Responsibilities
- Performs biopsychosocial evaluations in accordance with departmental policies and standards.
- Documents notes related to ongoing work in a timely and comprehensive manner.
- Collaborates with MD in planning and participating in family meetings.
- Supports and educates patients and families regarding illness, hospitalization, and post-discharge options and follow-up.
- Affords assistance in facilitating the continuum of care and accessing community services for patients and families as needed.
- Completes insurance reviews as necessary.
- Utilizes supervision by bringing appropriate materials for discussion and following up on recommendations.
- Participates in daily rounds and consults with other clinical staff to enhance patient care.
