Clinical Social Worker

Job Summary

You will be a compassionate advocate, providing vital support and guidance to individuals and families facing complex challenges. Every day, you will conduct psychosocial assessments, develop care plans, provide counseling and crisis intervention, and connect clients to resources. You'll advocate for clients and collaborate with multidisciplinary teams to ensure holistic support and promote well-being.

Responsibilities

• Counseling and crisis intervention
• Developing and implementing care plans
• Conducting psychosocial assessments and functioning evaluations
• Providing patient/family self-care management education
• Managing patient/family health management education
• Referring patients to bioethics and physician staff education
• Collaborating with multidisciplinary teams for holistic support
• Facilitating patient/family care conferences
• Coordinating continuity of care planning
• Supporting interdisciplinary communication and coordination
• Assisting with life care planning and palliative/end-of-life care
• Maintaining up-to-date resource lists and educating patients/families about available community resources
• Ensuring compliance with regulatory standards and evidenced-based practices
• Understanding organizational financial performance and managing revenue cycle

Requirements

• Master's Degree in Social Work from a Council of Social Work Education-accredited school
• Minimum of two years of clinical and/or medical social work experience
• Licensed Clinical Social Worker (LCSW) license in California

Benefits

The position offers a competitive salary range of $42.90 - $50.99 per hour.
