About the role
The Clinical Services Coordinator (CSC) acts as an initial contact point for providers and members, supporting medical management through phone or correspondence.
Responsibilities
- Conducting health risk assessments
- Coordinating care
- Assisting with system communications
- Data entry
- Supporting medical and disease management programs
- Arranging medical equipment
- Scheduling follow‑up appointments
- Facilitating referrals to community resources
- Ensuring effective care coordination
- Providing administrative support
- Performing member outreach
- Maintaining documentation
Qualifications
- High School diploma or GED
- At least 3 years of relevant experience, including 1 year in managed care
Skills
- Bilingual fluency in English and Spanish (preferred)
- Proficiency in MS Office (preferred)
- Knowledge of healthcare systems (preferred)
- Experience working with community resources (advantageous)
- Experience in integrated care models (advantageous)
