This position acts as a coordinator between internal and external referral sources, and acute inpatient rehab operations. The Clinical Rehabilitation Specialist is responsible for establishing relationships with case managers, physicians, payors, and other referral sources. They will ensure appropriate patient placement through clinical screenings and assessments in collaboration with both hospital Case Management and the Rehab Medical Director, and work closely with hospital case management to ensure a responsive and smooth flowing referral and admission process for the inpatient acute rehab program.

Responsibilities

  • Coordinate between internal and external referral sources and acute inpatient rehab operations.
  • Establish and maintain relationships with case managers, physicians, payors, and other referral sources.
  • Perform clinical screenings and assessments to ensure appropriate patient placement.
  • Collaborate with hospital Case Management and the Rehab Medical Director.
  • Ensure a responsive and smooth referral and admission process for the inpatient acute rehab program.

Requirements

  • Basic Cardiac Life Support must be obtained within 30 days of employment start date.
  • One of the following credentials: Registered Nurse (RN), Respiratory Therapist/Practitioner (RRT/RCP-State), Registered Respiratory Therapist (NBRC-RRT), Occupational Therapist (OT), Licensed Physical Therapist (LPT), or Speech-Language Pathologist (SLP).
  • Bachelor’s Degree or Associate Degree.
  • 2 years of experience required.
  • Prior acute rehab, subacute rehab, or LTAC experience preferred.
  • Prior Clinical Rehab Liaison or acute care hospital discharge planning experience required.
