About the Role

This position acts as a liaison between internal and external referral sources and acute inpatient rehab operations. The Clinical Rehabilitation Specialist is responsible for establishing relationships with case managers, physicians, payers, and other referral sources. They ensure appropriate patient placement through clinical screenings and assessments in collaboration with both hospital Case Management and the Rehab Medical Director.

Responsibilities

  • Assist and coordinate intake and pre-admission screening process.
  • Accurately document evidence of rehab admission criteria on Pre-Screen evaluation in a timely and legible manner.
  • Complete pre-screen updates within 48 hours of admission and communicate admission decisions to referral sources within 2 hours.
  • Provide information related to inpatient rehab and program features to patients and family members.
  • Communicate patient expectations and requirements for participation upon admission to the rehab program.
  • Offer each patient and family a tour of the rehab unit; coordinate tours if unable to provide personally.
  • Secure information relating to patients' resources and benefits.
  • Demonstrate clinical knowledge of rehab admission criteria, leading to appropriate identification of qualified rehab patients.
  • Advise referral sources on inpatient rehab criteria.
  • Function as the primary contact for external and internal case management and referring physicians.
  • Establish and maintain effective working relationships with all referral sources.
  • Complete face-to-face, in-service educational contacts with physicians, case managers, and other interdisciplinary members.
  • Serve as an educational resource to patients, family members, and caregivers.
  • Participate in market analysis, data analysis, and development and implementation of marketing and business plans for the Rehab Unit.
  • Contribute to the development and maintenance of referral source information such as brochures, flyers, referrals, and information kits.
  • Prioritize time management and meeting commitments to referral sources.
  • Develop a call-plan to prioritize utilization of time.
  • Assist in developing the annual strategic plan for the Rehab Unit/Hospital, including community outreach programs.
  • Ensure referral sources receive appropriate and timely follow-up communications regarding their patients and hospital programs.
  • Develop a backup system to handle calls when unavailable and ensure calls are handled 24/7.
  • Maintain reliable transportation to visit referral locations.
  • Assist as needed with PAI transmissions.
  • Perform on-site and off-site clinical evaluations to assist the Rehab Medical Director in determining patient appropriateness for admission.
  • Other duties as assigned.

Requirements

  • Licensed Professional in a clinical healthcare specialty (RN, Physical Therapist, Occupational Therapist, or Speech Therapist).
  • Current FL licensure or FL-approved compact license.
  • American Heart Association or Red Cross BLS certification.
  • Current state licensure in clinical practice area or eligible for state license transfer.
  • Graduate of an accredited school in clinical specialty.
  • Minimum 2 years of recent acute rehab clinical care experience.
