About the Role

The Clinical Registered Nurse (RN) II manages individualized patient care by promoting and restoring patients' health in accordance with medical and nursing plans of care and established policies and procedures. This role involves collaborating with physicians and multidisciplinary team members to develop, execute, and evaluate the multidisciplinary plan of care. The RN II assumes a full patient assignment and is responsible for planning, analyzing, intervening, and setting goals for patient care.

Responsibilities

  • Assessment:
    • Conducts individualized patient assessments, prioritizing data collection based on the patient’s immediate condition within specified timeframes.
    • Performs ongoing assessments as determined by patient condition and reprioritizes care accordingly.
    • Integrates nursing interventions into the multidisciplinary plan of care, incorporating standards of care and patient outcomes.
  • Planning:
    • Develops, modifies, and integrates an individualized care plan incorporating medical plans, interdisciplinary input, and standards of care.
    • Establishes measurable patient goals in consultation with the patient, family, and healthcare team.
  • Intervention:
    • Performs treatments as ordered by providers accurately and timely.
    • Delivers therapeutic nursing interventions as established by the individualized plan of care.
    • Recognizes and responds to urgent or emergent patient risks, initiating resuscitative measures as needed.
    • Manages care for a specialty patient population with accountability and responsibility.
    • Provides care in a non-judgmental, culturally sensitive manner, preserving patient autonomy, dignity, and rights.
    • Collaborates with interdisciplinary team members to ensure appropriate patient care interventions.
  • Evaluation:
    • Evaluates patient responses to care based on the effectiveness of nursing interventions relative to established goals.
    • Initiates changes in the interdisciplinary care plan based on evaluations.
    • Problem-solves with the specialty team to improve patient outcomes.
    • Documents patient assessment findings, responses to interventions, and progress toward problem resolution.
  • Patient Education:
    • Identifies learning needs of patients and families, adapting standard information to meet those needs.
    • Ensures patients and families demonstrate understanding within their abilities.
    • Communicates with patients and families to identify their needs and include them in the plan of care.
    • Supports patient and family determinations regarding treatment.
  • Self and Peer Learning:
    • Assesses and meets own learning and professional development needs.
    • Maintains current competency in nursing specialty and department requirements.
    • Attends required educational programs and may participate in additional workshops or professional societies.
    • Seeks opportunities for clinical development and is prepared to learn or currently functions in a charge nurse role.
  • Collaboration:
    • Makes sound judgments in coordinating increasingly complex patient care demands.
    • Supervises delegated care based on personnel abilities and patient acuity.
    • Reports changes in patient condition to appropriate personnel.
  • Service Recovery:
    • Participates in service recovery and uses appropriate resources during complex situations.
    • Handles conflict resolution in a timely and consistent manner.
  • Resource Utilization and Stewardship:
    • Supports organizational efforts for appropriate resource utilization (e.g., flexing during low census).
    • Serves as a preceptor or mentor, working directly with employees during orientation.
    • May assist in orienting, instructing, directing, and evaluating students assigned to the unit.
    • Is trainable for or currently functions in a charge nurse role.
  • Improvement Activities:
    • Participates in data collection for quality assurance activities if assigned.
    • Identifies and communicates recurring practice concerns in specialty areas and their impact on care.
    • Engages in initiatives resulting from nurse-sensitive, quality, and other specialty indicators or unit-based metrics.
    • Demonstrates utilization of Centra’s safety behaviors (e.g., Speak Up for Safety).
  • Research/Evidence-Based Practice/Quality Improvement:
    • May participate in research or evidence-based practice projects.
    • Communicates research and quality improvement data/findings to peers.
    • Interprets quality improvement data relevant to the clinical specialty and shares it with peers.
    • Contributes to developing or implementing methods to improve quality care in areas identified as deficient.
  • Ethics:
    • Maintains confidentiality as outlined in Centra policies.
    • Delivers care in a manner that protects patient autonomy, dignity, and rights.
    • Serves as a patient advocate.
    • Maintains professional role boundaries in patient-nurse relationships.
  • Performs other duties as assigned.

Qualifications

  • Education:
    • Minimum: Diploma or Associate Degree in Nursing.
    • Preferred: Bachelor of Science in Nursing (BSN).
  • Work Experience:
    • Minimum: One year of nursing (RN) experience.
  • Licenses & Certifications:
    • Minimum: Registration as a registered nurse in the Commonwealth of Virginia and/or holds a compact license to practice in Virginia.
    • Basic Life Support (BLS) certification.
    • Additional specialty requirements:
      • Advanced Cardiac Life Support (ACLS) for Cath Lab/Electrophysiology, Operating Room, Critical Care, Endoscopy, PACU, Interventional Radiology, Birth Center, Centra Specialty Hospital, Outpatient Ambulatory Surgery, and Plastic Surgery.
      • Pediatric Advanced Life Support (PALS) for Pediatrics.
      • Both ACLS and PALS for Emergency Department.

Pay

Salary Range: $32.92 - $51.02 per hour.
