Main duties of the job
- To complete medication histories and help achieve medication reconciliation targets in line with CQC recommendations.
- Counselling patients about their medication within pharmacy and at ward level.
- Communicate with a diverse range of people who visit pharmacy where there may be barriers to understanding, e.g., hearing impaired patients or patients whose first language is not English.
- Communicates with other professional and technical staff within the hospital or community pharmacy, regarding specific patient and operational issues.
- Supports other healthcare professionals, particularly the clinical pharmacists by identifying patients in any care setting who need a clinical medication review for a long-term condition, high-risk medicine, complex transfer of care, or problematic polypharmacy.
- Helps identify patients at risk of hospital admission or readmission (in accordance with guidance) and assists with referrals to community pharmacy for follow-up post-discharge.
- Undertakes an accredited technician checking course after completion of other in-house training schemes if not already held.
Person specification
- Mets All Band 5 Criteria
- Essential criteria:
- S/NVQ Level 3 in Pharmacy Services and approved underpinning knowledge e.g. BTEC Level 3 Diploma in Principles and Practice for Pharmacy Technicians or equivalent
- GPhC recognised pharmacy technician qualification
- Knowledge and understanding of all technical aspects of pharmacy patient services, supported by practical experience
- In addition, successful completion of an in-house competency-based training programme or previous relevant post qualification relevant clinical experience which provides an equivalent level of additional experience
- Practical experience of working in a relevant clinical pharmacy environment
- Excellent oral communication
- Excellent written communication
- Excellent customer care
- Working knowledge of pharmacy computer systems
- Desirable criteria:
- Medicines Management qualification
- Experience as an assessor/ Educational Supervisor
- An Accredited Accuracy Checking Qualification
- BTEC Level 4 Diploma in Clinical Pharmacy Services and Therapeutics
- Practical experience of working as a designated checker
- Familiar with Pharmacy stock control systems (e.g. JAC)
- Knowledge of current COSHH and Health and Safety Legislation
- Working knowledge of Microsoft word, excel, spreadsheets and databases
Applicant requirements
- You must have appropriate UK professional registration.
- The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
