About the role
Rush Medical Center Hospital, part of Rush University Medical Center, seeks a Clinical Pharmacy Specialist in Hematology/Oncology/SCT.
Responsibilities
- Outpatient care: Available as needed during the evaluation phase, participate in development of patient education materials, and be available on a consultative basis to the outpatient SCT clinic.
- Inpatient Acute Care: Attend interdisciplinary rounds, review drug therapy, reconcile home medications, discuss medication order clarifications, provide patient/family counseling, and assist with chemotherapy desensitizations.
- Discharge Planning: Participate in discharge planning, discuss relevant changes to the home medication list, and assist in assessing insurance coverage.
- Hematology/Oncology/SCT: Act as the pharmacy liaison, coordinate pharmacy participation in committees, provide updates on therapeutics, and participate in research and clinical resource management meetings.
- Department of Pharmacy: Provide updates on transplant therapeutics, assist with chemotherapy monographs, precept students, and promote rational drug therapy.
Qualifications
- Required: Bachelor of Science in Pharmacy, Illinois Pharmacist License, Post Graduate ASHP Year Two Pharmacy Residency in Hematology/Oncology, BCOP certification, BLS certification, ACLS certification, knowledge in intravenous admixture services, intravenous drug therapy, chemotherapy, and cellular therapies.
- PREFERRED: ACLS certification, Doctor of Pharmacy degree, and residency certificate.
