Job Purpose

The Clinical Pharmacy Specialist plays a vital role in delivering integrated, patient-centered care within designated outpatient clinics at the partner health system. This position focuses on ensuring appropriate medication therapy, implementing clinical protocols and guidelines, monitoring relevant medication therapy, and enhancing the quality and delivery of evidence-based medicine. The Clinical Pharmacy Specialist works on-site (Monday through Friday daytime shift hours) integrated with the primary care team and collaborates closely with both hybrid and onsite pharmacy service teams to maximize clinical services and coordination of care.

Job Responsibilities

  • Conduct comprehensive assessments for disease management under collaborative practice agreements or clinic protocols.
  • Work on-site in close coordination with interdisciplinary teams, including both in-person and hybrid care models.
  • Auxiliary in the development, maintenance, and appropriate use of treatment plans and supportive care plans in alignment with clinical goals and patient needs.
  • Monitor and evaluate medication regimens to ensure clinical criteria are met, including therapeutic goals, adverse effects, and drug interactions.
  • Ensure all clinical criteria are in compliance with FDA standards, national clinical guidelines, and appropriate evidence-based literature.
  • Identify, resolve, and prevent medication-related problems to ensure safe and effective pharmacotherapy.
  • Provide direct, individualized counseling on medication use, lifestyle modifications, and disease state education.
  • Address barriers to adherence, including cost, access, and health literacy, through targeted interventions.
  • Willing to do whatever is necessary for the betterment of the organization.

Licensure & Education

  • Doctor of Pharmacy (PharmD) and active Louisiana pharmacist license required.
  • Oncology Pharmacy Specialty Certification (BCOP) required within 1 year of hire.

Experience

  • PGY1 + PGY2 residency in area of specialty/related field
  • OR PGY1 + 1 year practice experience in area of specialty/related field
  • OR 3 years’ experience in area of specialization

Skills & Expertise

  • Strong clinical judgment, communication, and process management.
  • In-depth knowledge of pharmacology, FDA regulations, and national treatment guidelines.
  • Proven ability to ensure legal compliance and deliver high-quality, detail-oriented care.
  • Experience with EPIC & Beacon Oncology treatment plans preferred.
